The Quality of Life Questionnaire measures the relationship between a client’s qualify of life and other behaviours or afflictions, such as physical health, psychological health and alcohol or other substance use.  Results highlight areas of the client’s life that may require change to alleviate specific symptoms.  Its efficient design makes the QLQ an ideal screening tool for employee assistance, wellness, stress, weight control or any other programs in which people desire change.

This 192-item measure consists of five major domains, 15 content scales and a social desirability scale.  The five major domains are:  General Well-Being, Interpersonal Relations, Organisational Activity, Occupational Activity and Leisure and Recreational Activity.


The QLQ consists of a reusable Question Booklet and Forms for recording the client’s responses.  Scoring is quick and easy.  The client’s responses transfer through to a hidden scoring page where instructions guide you through simple calculations to obtain a Total Quality of Life score.  A Profile Form is also included as part of the Form where raw scores are converted to T-scores.

The QLQ  is also offered in online format for use with MHS Scoring Organiser.  MHS Scoring Organiser is an internet-based client management program and is free with your purchase of MHS online reports.  MHS Scoring Organiser helps you manage all your MHS online asssessment tools.  You can instantly generate reports and customise them to suit your purpose.  You may then save them, print, export or integrate the results with your MHS Professional Tool Suite software programs.  You may also track your report uses.
